Montrose Support Coordination is now part of Northcott, this services is delivered by the same excellent team of Support Coordinators from our Queensland centres.
Support Coordination is available across South East Queensland including Brisbane South, Brisbane North, Logan, Ipswich, Toowooomba, Sunshine Coast and Gold Coast.

What is Support Coordination?
Support Coordination is designed to assist you in finding the best services to deliver the outcomes in your NDIS plan. There are three levels of Support Coordination available under the NDIS. Your NDIS planner will talk to you about which level of Support Coordination is most appropriate for you.
There are 3 levels of Support Coordination:
- Support Connection – Short term assistance designed to build your capacity to manage your own plan.
- Support Coordination – Longer term support for more complex support needs. This level of support focuses working together to understand the funding, what you expect from services, and how you want this to be designed.
- Specialist Support Coordination – similar to level 2 but with more intensive support.

Get the most out of your NDIS plan. Northcott’s Coordination of support service is an independent service that assists participants and their families with multiple or complex support needs to understand and implement their NDIS Plan and access services from a range of providers.
Personalised Support Coordination
We specialise in children, families and young adults but works across the age spectrum. Our specialist Support Coordinators can work with people from 0 to 65 years (or 65+ who have NDIS funding package) including:
- Help you understand your NDIS Plan
- Work with you to design Support Approaches
- Identify services that meet your individual needs and those identified in your NDIS plan
- Establish and coordinate your service providers and support
- Help in a crisis: Planning, Prevention, Mitigation and Action
- Build Capacity and Resilience
- Monitor your NDIS plan and review your goals prior to a plan review
- Report to the NDIA
Frequently asked questions about Support Coordination
Yes, Support Coordinators need to manage ‘Conflict of Interest’ by remaining completely impartial for the participant. They offer a choice of all suitable services available from a range of disability service organisations.
Support Coordinators charge in 15 mins blocks of time for direct customer work like meetings and indirect customer work such as administrative tasks, setting up appointments, reports, research, liaising and travelling to and from a customer.
No. If you want NDIA to speak with you about a participant, you have to organise a Third Party Consent form to exchange information in your favour.
