Individual Services are services where you receive individual (one-on-one) support to achieve a goal or outcome in their home, the community or at your workplace.
You’ll be matched with a trained and experienced support worker who can meet your support needs. You can choose when and where they receive individual services, with activities such as personal care, social outings, transport, shopping, doctor’s appointments and cooking.
NDIS individual support funding

Yes, Northcott is a registered NDIS provider, offering NDIS services and supports for both adults and children with significant disabilities, in achieving their goals and enhancing their quality of life.
Our services encompass a wide range of supports aimed at enhancing your independence, wellbeing social participation, independent living, community access, education, building your skills, control over your funding and management of daily living for people with disability. Northcott’s individual support services can cater to your individual needs and take into consideration what is outlined in the participant’s NDIS plan.
An NDIS participant can access their funds to receive assistance with a range of individual supports like at home care, social participation, funding support, employment support, respite, allied health and clinical services and much more.
Northcott delivers quality services tailored to each participant’s goals and abilities, ensuring reasonable and effective support implementation. Northcott works closely with participants, their families, and relevant stakeholders to ensure that NDIS funds are utilised appropriately and in line with the participant’s plan. As a registered NDIS provider, Northcott has the expertise and resources to deliver supports and services that enable individuals to achieve their goals, manage their lives, and participate meaningfully in their community.
With Northcott’s individual supports, you can achieve a range of goals tailored to your specific needs and preferences under the National Disability Insurance Scheme (NDIS). Here’s some of our individual support services and the goals you can achieve with them:
Support Coordination
With Support Coordination, you’ll receive assistance to be able to take control of your NDIS plan so that you can make informed decisions, work towards your goals, and connect with the right NDIS providers, along with community, mainstream and other government services.
Individual Support
With individual support, you’ll be matched with a trained and experienced support worker/carer who can meet your support needs. You will be able to choose when and where you receive individual services within your local area or at home, with activities such as personal care, household tasks, social outings, transport, shopping, doctor’s appointments and cooking. You will learn to build your independence and not need to rely on family, learn social skills and take control of your life.
Social Work
With Social Work, you will receive support for you and your family to deal with daily challenges. Our Social Workers will work with you to support you to access community resources, access other service providers and support you with counselling. You’ll be able to have improved relationships, help with managing stress and support for you and your family.
With Northcott, you can access a comprehensive range of individual support services aimed at enhancing your ability to participate fully in society, manage daily life, take control of your health and wellbeing and achieve your goals, ultimately leading to a more fulfilling and independent life.
We focus on person centred active support. This is an approach that is fundamental to the provision of effective, person-centred support. It is a way of providing assistance to people that focuses on making sure individuals are engaged and have the ability to participate in all areas of their life.
The practical outcome of person centred active support is better engagement. This means we explore ways of supporting you the way you want to be supported, with guidance from you, your family, community group and anyone you want us to connect to. We don’t believe in a rigid system with broad solutions that compromise our level of service. Hence the way we manage the delivery of our group services depends on our clients, disability supports needs, interests and engagement. For example, many of our Recreation groups plan activities as a team, manage the social calendar, research businesses and use their skills to create a personalised event plan for their group.
