Therapy (Fee for Service) – Service Disclosure Statement

These are the Terms and Conditions for the Therapy service you have purchased.

  1. What if I need to schedule, reschedule, or did not attend my service?
    1. We are available between 8am – 8pm on Monday to Friday, and 8am – 4pm on weekends and public holidays to assist you with scheduling or rescheduling your services.
    2. To schedule or reschedule your service, email [email protected] or call 1800 818 286.
    3. You will need to give 24 business hours notice if you wish to reschedule your service. Fees may apply if you do not notify us within 24 business hours.
  2. How do I pay and what is included in my Therapy service?
    1. We require you to pay upfront with our Customer Service Team on 1800 818 An invoice will be sent to your nominated email account.
    2. If you require us to travel to you, this charge will be added on top of the quoted Therapy hourly rate.
    3. The hourly Therapy rate will include time for report writing and developing resources for your session.
  3. How can I give compliments, suggestions or make a complaint?
    1. We welcome your compliments, suggestions and/or complaints about services. We will listen to your compliments and suggestions, and respond to complaints fairly and in a reasonable amount of time.

Northcott will not use or share your personal information with anyone without your consent, unless we have concerns for your safety or we are required or authorised to by law. For example Mandatory Reporting of Child protection concerns.
