About The Role

  • 4 Days - Permanent Part time role
  • Wollongong based location with Illawarra caseload

As a Support Coordinator, you will be responsible in ensuring our customers are supported to achieve their goals as identified in their NDIS plans, which will enable greater independence!

You will build capacity and implement all supports in a participant's NDIS plan, as well as report back to the NDIA on the outcome and goals as required.

In this role you will:

  • Work with NDIS participants, families and other stakeholders to understand, implement and monitor NDIS plans.
  • Report on participant outcomes to the NDIA, as requested by the agency.
  • Coordinate supports to assist individuals and families within their homes and varying communities.
  • Collaborate and liaise with internal and external service providers.
  • Have a demonstrated understanding of NDIS and experience managing funded plans.

About you

Ideally, you will hit the ground running but just a few things to mention that we desire are:

  • A demonstrated experience in interpreting and implementing NDIS plans by working with services and organisations within the local general community.
  • The ability to communicate effectively with a diverse range of people to build trust and develop rapport.
  • Experience working with customers with complex support needs which include Mental Health/Psychosocial disabilities.
  • Current Australian Driver's licence.

The successful candidate will also be required to clear probity checks including a National Criminal History Record Check and Working with Children Check prior to commencing employment.
