About the opportunity

Reporting to the Area Manager, this is a fantastic opportunity for someone who has Support Coordination and leadership experience to be part of one of Australia's largest not-for-profit disability services providers. As a Team Leader, you will maintain a small customer caseload whilst supporting your team to assist customers in maximising their access to services within their allocated given budget.

 In this role you will:

  • Provide management, leadership and mentoring for the Support Coordination team.
  • Collaborate and liaise with internal and external service providers.
  • Provide guidance and feedback to management that assists with continuous improvement and growth of the service.
  • Monitor, review and report back to the NDIA on the outcomes and goals, as required.
  • Coordinate supports for individuals and families within their homes and in the community.

What we are looking for:

  • A minimum of 12 months’ experience working as a Support Coordinator.
  • Demonstrated experience leading and motivating a team.
  • Demonstrated experience in providing support using enablement and strengths-based approaches.
  • Previous experience supporting people with complex needs and challenging behaviours including, mental health, ABI and cognitive disability.
  • Driver's licence; travelling within the region will be required.
  • Understanding of the NDIS and associated legislation including the NDIS Quality and Safeguards Commission procedure.
  • Experience using the Carelink system will be highly regarded.
